The Millicent quest line in Elden Ring is one of the more complex quests in the game. Aside from having you travel all over the Lands Between, you’re going to be faced with a decision near the end of the quest. If you’re wondering how to finish the Millicent quest in Elden Ring, read on to find out more.
How to start Millicent quest
Before we get into it, we highly recommend that you’ve already made a lot of progress in Elden Ring. This is because this quest has you running around all over the Lands Between. And the Millicent quest line will be easier to navigate if you’ve already unlocked most of the Sites of Grace in the game.
For players to begin this quest, you need to speak with Gowry. He can be found in a shack in the southern part of Caelid. Once you speak to him, make sure to finish his dialogue to begin the quest.
Gowry tells you he needs your help in healing “a certain young girl.” But before you can do that, you need to retrieve the Unalloyed Gold Needle first.
How to retrieve the Unalloyed Gold Needle
For you to retrieve the Unalloyed Gold Needle, you need to fight a boss near the Inner Aeonia Site of Grace as pictured above. After getting here, you’ll fight Commander O’Neil near the eastern edge of the swamp.
Commander O’Neil is not hard to defeat on his own. But the enemies he summons to help him are what makes this boss fight rather challenging. If you’re finding it hard to defeat him, you can use the Scarlet Rot nearby and lure him there. The Scarlet Rot can help by inflicting damage on him which will make it easier for you to finish him off.
After defeating him and getting the Unalloyed Gold Needle, head back to Gowry’s Shack. In exchange for the Unalloyed Gold Needle, Gowry will give you Sellia’s Secret.
Then, fast travel someplace else or just reload the game before talking to Gowry. Afterwards, he will give you the repaired needle.
Find Millicent at the Church of the Plague
After you get the repaired needle from Gowry, he proceeds to tell you Millicent’s location. The church he’s referring to is the Church of the Plague just east of Gowry’s Shack.
If you still remember the quest item you received from Gowry called Sellia’s Secret, you can use it here. Light the beacons in the village to access the road.
Millicent is located inside the ruins at the Church of the Plague. After talking to her, give her the needle and rest at the Site of Grace near you.
Then, talk to Millicent again and exhaust her dialogue until she gives you the Prosthesis-Wearer Heirloom.
Return to Gowry’s Shack again and reload the game so you’ll see Millicent there. After talking to her, make sure you finish her dialogue to proceed to the next part of the quest.
Millicent can be found at Altus Plateau next
Millicent’s next location is near the Erdtree Gazing Hill Site of Grace. However, you won’t be able to continue with the quest unless you’ve already obtained the Valkyrie’s Prosthesis.
To get this item, you need to open a chest in the Shaded Castle. But before you can get the chest, you’ll encounter a Cleanrot Knight that you can either fight or avoid.
The chest is located in the room behind the Cleanrot Knight. You can instantly obtain the item after opening the chest.
After getting the Valkyrie’s Prosthesis, you can make your way to Millicent. She is located north of the Erdtree Gazing Hill Site of Grace. Give her the item and exhaust her dialogue again to proceed to the next part of her questline.
Head to Windmill Village to encounter Millicent again
Of course, you’ll have to fight another boss before encountering Millicent in this quest again. In this area, you’ll fight against the Godskin Apostle. To find this boss, just go to the area near the windmill at the top.
But be ready since this boss is a bit difficult to defeat. You need to be quick on your feet in dodging. So, study this boss’ attack patterns before going in with dealing damage. After defeating the Godskin Apostle, you’ll get a few rewards but you shouldn’t forget to activate the Windmill Heights Site of Grace.
If you rest or reload the game, you’ll now see Millicent standing near the windmill. Just like in previous encounters, make sure to finish her dialogue to proceed to the next part of the quest.
Meet Millicent at the Mountaintop of the Giants
To speak with Millicent again, look for her at the Ancient Snow Valley Ruins Site of Grace. This location can be found south of Castle Sol.
After you talk to Millicent here, she mentions something about calling her in battle. Moreover, she tells you about the Haligree Secret Medallion which is used to unlock a hidden area at the Grand Lift of Rold.
Go to Elphael, Brace of the Haligtree
Millicent’s next location is the Prayer Room Site of Grace in this area. But before you enter the Prayer Room, make sure to defeat the enemy standing outside. This is to ensure that Millicent isn’t harmed by this enemy once you talk to her.
After taking care of the enemy, make your way in the Prayer Room where you’ll find Millicent. Speak with her and make sure to exhaust her dialogue.
Fight the Ulcerated Tree Spirit
To progress further into this quest about Millicent, you need to fight another boss called the Ulcerated Tree Spirit. This boss is located near the Drainage Channel Site of Grace.
This boss is not easy to fight since they spawn in a pool full of Scarlet Rot that can damage you after a while of standing in it. So, we recommend you using ranged attacks or being mindful of the gauge that slowly fills as you’re inflicted with Scarlet Rot.
After you kill the Ulcerated Tree Spirit and collect the rewards, rest at the Drainage Channel Site of Grace.
Look around the area for a gold and red summon sign
In this part of the quest, you now have to make a choice that ultimately affects the quest’s ending. The gold sign asks if you want to assist Millicent in fighting against invaders.
On the other hand, the red summon sign gives you the chance to betray Millicent and fight her yourself.
Be summoned to assist Millicent
If you choose this option, you’ll help Millicent fight against four invaders. But be careful as you don’t want the enemies doing too much damage to Millicent and eventually killing her. You also need to be careful to not jump on Millicent since this can instantly kill her.
After you successfully defeat the invaders, you’re rewarded with the Rotten Winged Sword Insignia talisman. Moreover, reloading the game or resting lets you interact with Millicent’s deceased body. Looting her will give you the Unalloyed Gold Needle item again. This time, you can use it to get Miquella’s Needle to get one of the endings in Elden Ring.
Be summoned to challenge Millicent
If you choose this option, you are essentially betraying Millicent. After fighting and defeating her, the quest ends here.
You’ll obtain Millicent’s Prosthesis after killing her but you can’t get the Unalloyed Gold Needle anymore.
Go back to Gowry’s Shack
After choosing either helping or betraying Millicent, make your way back to Gowry. If you betray Millicent, he will already be dead by the time you get there.
But if you choose to assist Millicent, Gowry will be alive in his shack although he seems to have lost his mind when you arrive.
The only option you have is to kill him so you get the Flock’s Canvas Talisman item. This item is also obtainable if you kill Millicent.
